OUT OF NETWORK SERVICES OR IF YOU ARE NOT USING INSURANCE, my fee is: $750 for the initial consultation, $285-525 for follow up visits (varies with time or complexity of care).
I am out of network for HIP, Aetna, Blue Cross, Anthem, Horizon, Oxford, UHC, Oscar, Affinity, First Health, Medicaid, & Medicare. As of 3/1/25, I will also be out of network for all Carelon plans including GHI, NYSHIP & SAG/Aftra.
I am in network and currently accepting new patients with CIGNA.
I am in network and accept 1199 for existing patients only.
For in network care, you are responsible for meeting your deductible, and co-payments. Payment is due at the time of service.
